“The Public Reporting Website maintained by the Ministry of Health
and Long Term Care contains historic information which may relate
to resolved issues which could be well over one year old. Accordingly
the information is not reflective of the care and services provided
at Trillium Villa
currently. All issues identified by the Ministry of Health
and Long Term Care are resolved immediately and our plan
of action to resolve such issues is provided to the Ministry. Identification
of issues and their resolution is part of our Internal Quality
Process. The friendliness, warmth and excellence of care at Trillium

Results of our most recent Annual Inspection by the Ministry of Health
Our last Annual Inspection was held on July 14-17,2009. One unmet
standard (B3.16) was issued which relates to safety risk (please see
below for the response to this unmet standard)
Unmet Standards for Reporting Period
B3.16 Each resident’s environmental
shall be maintained to minimize safety and security risks.
This unmet criteria was
issued due to utility room and hair salon doors that were not locked. As
a result, keypad entry locks were installed on all utility room doors
and the hair salon is now also locked when unattended. This standard was
put back into compliance during a follow-up inspection in February 2010.
RESOLVED.
B1.3 Building on previous
information, medical and nursing assessments shall be completed with 7
days of admission.
This unmet criteria was
issued as a result of no documentation regarding the follow up of a
newly admitted resident’s condition that had been noted on their
pre-admission medical history. Subsequently, we have put in place an
auditing system to check through each new admission’s medical history
and ensure that any follow up is documented in the resident’s chart.
