The Public Reporting Website maintained by the Ministry of Health and
Long Term Care contains historic information which may relate to resolved
issues which could be well over one year old. Accordingly the information
is not reflective of the care and services provided at Afton Park Place
currently. All issues identified by the Ministry of Health and Long
Term Care are resolved immediately and our plan of action to resolve
such issues is provided to the Ministry. Identification of issues and
their resolution is part of our Internal Quality Process. The friendliness,

Results of our most
recent Annual Inspection by the Ministry of Health
On February 5-8,
2008 we had our annual in-depth review with no unmet standards being
issued. This shows our commitment to the Ministry of Health and Long
Term Care, our residents and our home.
Unmet Standards
for Reporting Period
The following
standard was identified by the Ministry of Health and Long Term Care
Compliance Advisor as being unmet based on a complaint visit in
August of 2007.
B3.12 Each
resident shall have opportunities and assistance to participate in
programs which are appropriate to his/her interests and preferences
both within the facility and in the community.
Based on this
concern, a policy related to resident goal setting was developed. A
review of assessments and reporting was completed with the staff of
that department.
RESOLVED as of
our October 2007 annual inspection.

There
