“The Public Reporting Website maintained by the Ministry of Health and
Long Term Care contains historic information which may relate to resolved
issues which could be well over one year old. Accordingly the information
is not reflective of the care and services provided at Afton Park Place
currently. All issues identified by the Ministry of Health and Long
Term Care are resolved immediately and our plan of action to resolve
such issues is provided to the Ministry. Identification of issues and
their resolution is part of our Internal Quality Process. The friendliness,
warmth and excellence of care at Afton Park Place will be clearly visible

Results of our most
recent Annual Inspection by the Ministry of Health
Afton Park Place had its last annual in depth review in May 2009.
There were 4 unmet criteria as a result of that visit. All were
back in compliance when visited by the MOH Compliance Branch in
December of 2009.
Verified Concerns for the Reporting Period above:
There were two verified concerns in this period; resulting in two
unmet criteria being issued as a result of visits in July 2009 and
April 2010.
B1.6 Each resident’s care and service needs shall be reassessed
at least quarterly and whenever there is a change in the resident’s
health status. This prompted a review of policies, protocols
and accountabilities. RESOLVED

All written complaints, including follow up action are to be sent
to the LTC Division. The Home is very proactive about
dealing with all complaints – verbal and written. Copying of
investigative reports to the MOH & LTC Compliance Branch has been
added to our protocol.
